Registration Forms

Please be sure to download, print, and complete your Participation Application, your Participation Contract, and your Emergency Contact & Medical Info form and bring them with you to the first session. For your child’s protection – especially in case of an emergency – we must have all three on file before your child will be allowed to participate in any Prestige Basketball Clinics activities. Your child will not be allowed to participate without them under any circumstances.
